What Will Happen During My First Visit?

One of our team members will conduct a thorough history of medical and dental health and a thorough examination to assess the bone loss, assess for loose teeth, bite, TMJ problems, oral cancer screening, and other signs of periodontal disease.

Then Dr. Dominik Dubravec will discuss therapy alternatives with you. All your particular concerns and questions will be discussed in detail.

What to Bring to Your Appointment

Please help us with the following information on your first visit:

Your referral slip and x-rays from your referring dentist
A list of medications you are currently taking (please note that you can fill out your patient registration and medical history form on our website)
If you have dental insurance, please bring any forms or insurance cards with you to the appointment

Please notify the office if you have a medical condition or concern prior to surgery (e.g. artificial heart valves or joints, heart murmur needing premedication, severe diabetes, or hypertension).

We do our best to provide you with accurate coverage estimates based on information available to us. Dealing with insurance companies can be difficult and time consuming. In an effort to maximize your dental benefits, we ask that you keep us informed of any change to your insurance. It is important that all information about you and your insurance is current. We accept both medical and dental insurance plans and we do the filing.
